【摘要】目的：探討喉癌術後患者情緒障礙的心理 社會 因素。病後心理狀態以及經西酞普蘭 治療 後心理狀態的對照。 方法 ：60例喉癌術後出現情緒障礙患者， 應用 HAMD、HAMA、SDS進行問答調查，並隨機分為對照組與治療組各30例。治療組經西酞普蘭治療6周後，與對照組進行心理狀態比較。結果：治療組和對照組的HAMD、HAMA、SDS總分均有顯著性差異，其副反應的發生率低和嚴重程度輕。結論：西酞普蘭治療喉癌術後情緒障礙患者安全有效。

Comparing study on Citalopram in tranting Emotioal Disorder
of Postoperation of Laryngeal Carcinoma
( The Second People's Hospital of Shantou, Guangdong Shantou 515041, China)Abstract: Objective: To approach the psychosocial factor of laryngeal carcinoma patients emotional disorder, compare the mental status of post-illness and who have treatd with citalopram. Method: 60 patients respond examination on HAMD、HAMA、SDS of laryngeal carcinoma with emotional disturbance, dinded in to control group and treatment group, each group has 30 patients , the treatment group were treatd with citalopram for 6 weeks, compare the mental status between two groups. Result: The totle score of HAMD、HAMA、SDS were significantly different between control group and treatment group, the side effect of citalopram incidence rate was low and the severity extent was light. Conclusion: Citalopram is safety and utility for healing the emotional disorder of the patients who have acecptd the laryngeal carcinoma operation.

喉癌術後患者出現情緒障礙，嚴重 影響 患者的生活質量。情緒障礙是精神領域的重要 研究 課題，對喉癌術後抑鬱已有較多報道。但將焦慮、患者對疾病和對疾病治療態度與抑鬱一併研究治療的報道較少，焦慮、抑鬱在器質性疾病中既相互聯繫又彼此獨立，值得進一步研究。
1 對象和方法
1.1 研究對象:
選擇我院耳鼻咽喉科1998年2月至2006年6月喉癌患者住院手術後出現情緒障礙患者60例，其中男56例，女4例，年齡45～78歲，平均年齡59.69±13.20歲，排除標準：①過去有精神疾病史；②意識障礙或其他腦部疾病的；③治療不合作者；④有嚴重心臟病，肝、腎功能損害者。
1.2 研究方法
1.2.1 用HAMD（抑鬱量表）HAMA（漢密頓焦慮量表）SDS（抑鬱自評量表）［1］及喉癌病人情緒調查問卷進行評定和記錄。調查 內容 包括病人一般資料，軀體疾病診斷和治療情況。病人對疾病態度及對醫生治療的信任程度等。
1.2.2 治療方法:
把符合入組條件的60例患者隨機分為對照組及治療組各30例，治療組給予西酞普蘭10mg/d，2周後加至30 mg/d，在治療6周後與對照組比較。
1.2.3 治療前、中、後進行三大常規，肝功能及心電圖、腦電圖監測。
1.2.4 統計學處理:治療組與對照組HAMD、HAMA、SDS評分比較。病例情緒障礙程度比較，統計數字用±s、並用t檢驗（統計軟體SPSS 11.5）。
